Job Description:
As a Registered Nurse After-Hours Weekend Hospice at Agape Care
Group, you are at the forefront of delivering exceptional, tailored
care to patients in their most vulnerable times. This challenging
yet rewarding position involves working weekends to provide
continuous, compassionate, and skilled nursing care for hospice
patients during after-hours. Our ideal candidate is a dedicated
healthcare professional who thrives in emotionally intense
situations and demonstrates profound commitment to enhancing
patient comfort and dignity. Provide direct patient care to hospice
patients within their own homes during after-hours and weekends.
Assess, plan, implement and evaluate care plans in consultation
with healthcare providers according to patient-specific needs.
Administer medications, perform diagnostics, and monitor vital
signs as per the tailored care plans. Support patients and their
families emotionally, manage crisis interventions and provide
education on end-of-life care. Collaborate with a multidisciplinary
team to facilitate cohesive care and optimal outcomes. Document
patient care services by charting in patient and department records
following legal and accreditation guidelines. Strong clinical
skills with proficiency in pain management Excellent communication
and interpersonal skills Ability to work autonomously and make
decisions in high-pressure situations Solid understanding of safety
protocols and respect for confidentiality and legal aspects of

Current Registered Nurse (RN) license in the state of practice CPR
certification Certification in palliative care nursing (CPN)
