Job Description:
Agape Care Group is seeking a dedicated and compassionate Nurse
Practitioner to join our team, providing unparalleled hospice and
palliative care across the Southeast. In this role, you will work
closely with patients and their families, offering clinical and
emotional support during the pivotal phases of their healthcare
journeys. Our Nurse Practitioners are crucial in crafting care
plans that honor the values and meet the needs of each patient,
ensuring their comfort and dignity. Conduct comprehensive patient
assessments and establish appropriate care plans. Collaborate with
physicians, nurses, and other healthcare professionals to deliver
effective multidisciplinary care. Provide palliative care
consultations and manage symptom control to enhance the quality of
life for patients. Educate patients and family members on disease
progression, treatment options, and end-of-life care. Monitor
patient conditions regularly and adjust treatment plans as
necessary. Ensure compliance with healthcare regulations and
ethical standards in the delivery of care. Participate in the
training and professional development of nursing staff. Strong
clinical skills in a hospice or palliative care setting Excellent
communication and interpersonal abilities In-depth knowledge of
disease pathophysiology and complex symptom management Ability to
make critical decisions regarding patient care Empathy and a deep
commitment to patient advocacy Medical Insurance Dental Insurance

licensure in the state of practice Board certification in Hospice
and Palliative Medicine (HPM) is highly desirable Valid driver’s
license and reliable transportation for in-home patient visits as
necessary
