Job Description:
Agape Care Group is seeking a compassionate and dedicated Nurse
Practitioner to join our Palliative Care team. As a key member of
our healthcare services, you will play a pivotal role in enhancing
our patient-centered approach, providing advanced nursing care in
palliative settings to meet the unique needs of patients
experiencing chronic illnesses or life-limiting conditions. Based
in our Southeast locations, this role encompasses both clinical and
empathetic skills to ensure our commitment to dignity, comfort, and
quality of life is upheld in every patient interaction. Conduct
comprehensive patient assessments to determine their clinical
condition and palliative care needs. Collaborate with a
multidisciplinary team to plan and implement effective palliative
care strategies. Prescribe medication and other therapies in
accordance with patient health status and care goals. Offer
guidance and counseling to patients and their families about the
nature of illness, prognosis, and available care options. Monitor
patient progress and modify treatment plans as necessary to ensure
optimal care. Document patient care activities and maintain
up-to-date patient records in accordance with legal and ethical
standards. Provide education and support to healthcare staff on
palliative care principles and practices. Strong clinical skills in
a healthcare setting Excellent communication and interpersonal
skills Ability to work collaboratively in a team-oriented
environment Strong critical thinking and problem-solving skills
Mastery of pain and symptom management Compassionate and

Nurse and Nurse Practitioner in the state of practice Board
Certification in Palliative Care Nursing (CPNP-PC) preferred
Eligible or completion of a fellowship in palliative care is an
advantage
