As our RN Care Navigator, you'll be fulfilling all requests for resident Care Navigation billable services. This position will serve in a nursing role guiding the health needs of residents in The Apartment and Cottage Community, Myers Hall, Arcadia, The Estes Community and Christie Rehab in order to enhance and promote their highest level of functioning and quality of life.
A Typical Day:

• 
  Serve in a nursing capacity to consult and guide residents through healthcare decisions.
  
  
• Work collaboratively with the Care Navigator to meet all requests from residents.
  
• Arrange and maintain a flexible schedule of appointments with the Care Navigator.
  
• Follow through with established short and long-term care plans to promote emotional security, mental support, and environmental safety.
  
• Ensure that all resources needed to complete an appointment are arranged in a timely fashion.
  
• Record and turn in billing weekly for clients using care services.
  
• Communicate any resident concerns to the Care Navigator.
  
• Serve as a resource and communication mechanism for family members when billable Care needs arise.
  
• Drive residents to social, medical and other requested appointments.
  
• Maintain safe practices and plan for residents with varying levels of cognitive and functioning abilities.
  
• Monitor, assess and convey level-of-care or transfer considerations to the Care Navigator.
  
• Perform new client assessments for BG at Home, BG's home care service.
  
• Conduct reviews, assessments and all necessary paperwork for Long Term Care insurance clients within the Bishop Gadsden community.

  What We Look For In A Candidate:
  
  • 
    
  • Active Registered Nurse license in the state of South Carolina
    
  • College degree or equivalent combination of education and experience.
    
  • Proficient with computer programs.
    
  • Knowledge of Senior Services and Resources.
    
  • Valid SC driver's license in good standing
    
  • 
    Ability to communicate effectively, verbally and written, with residents, families, and staff.
    
    
  • Ability to convey a sense of caring and respect to residents and families, regardless of circumstances.
    
  • Basic computer skills and working knowledge of Microsoft Office programs (Word, Excel, etc.).
    
  • Ability to walk, stoop, stand, bend, reach and lift (up to 50 lbs.).
